These are the official forms a family plan already links to. Each one opens on the agency's own page or PDF.
SSA-1696, Appointment of a representative
This lets the loved one name someone to handle the claim with Social Security. You can fill the paper form, or start it online with that person.
SSA-3380-BK, Function report from the family
This is the family's account of the loved one's hard days. You can complete it even when the loved one cannot.
Social Security's SSI application page
This page explains how to start SSI by phone or online, and how the date of the call can count as the filing date.
SSA-8000-BK, SSI application
This is the application for Supplemental Security Income. Social Security staff usually fill it out with you, in person or by phone.
SSA-3368-BK, Adult disability report
This report tells Social Security about the loved one's health, treatment, and work. You may be able to complete it while you apply online.
SSA-16, Disability insurance application
This is the application for disability benefits on the loved one's own work record. You can start it online, by phone, or at an office, and the paper form is still current.
SSA-827, Permission to share records
This lets doctors, schools, and others send the loved one's records to Social Security. The loved one signs it in ink.
Medicaid application, Illinois example
This example is for the sample family in Illinois. Lark County is not a real county, and your own state will use a different site. You can apply online, or call 800-843-6154.
IL 444-2998, Approved representative form
This lets a family member receive the Medicaid notices and help with the application. The applicant and the representative both sign.
SSA-8012, Household expenses and contributions
This is the current statement of housing costs and what the loved one pays toward them. Social Security may ask someone in the home to fill it out and send it back.
